The pneumococcus may reach the brain via hematogenous or direct spread. Once there, the pyogenic coccus induces a purulentent exudate, containing much fibrin. This fibrin may impede the circulation of the CSF, leading to hydrocephalus. Often, the exudate is worse in the dependent areas of the brain.
