CMV inclusions are both intranuclear and intracytoplasmic, but the nuclear features predominate. The nucleus is enlarged (as is the overall cell), it is pyknotic (dark), and has a prominant halo (so-called owl's eye). This combination of features is pathognomonic for CMV, and it can be diagnosed from histologic appearance alone.
